DISK_ADDR_TYPE_SATA, DISK_ADDR_TYPE_IDE and DISK_ADDR_TYPE_SCSI are
driven by basically identical data types. Unify them. Note that
changes to 'str2DiskAddress' are deliberately lazy as it will be
refactored later.
