That did lead me to wonder what happened to server->nvmfds - there are
two references to it in the code and both have been removed, but it
turns out it was only ever being written to, which I guess made it less
than useful.
The nvmfds stuff was an optimization used with the old integrated event
loop, which I forgot to remove in my first patch. It just let us know
how big an array to allocate for poll_fd, which is unneccessary now
that we have a general purpose isolated event loop.
